1. List of creditors; including creditors name, address, account number and account balance
2. Drivers License
3. Social Security card
4. Tax returns for the last year
5. Current pay-check stub
6. Evidence of income for the past 6 months (pay stubs, etc.)
7. List of Monthly Expenses
8. Any documents pertaining to foreclosure or any other legal action
